Course Details

This course introduces the principles and practical application of Root Cause Analysis (RCA) to support effective problem solving and sustainable corrective action. Participants will learn how to identify true root causes using structured tools such as the 5 Whys, Fishbone Diagrams, Brainstorming, and Flowcharting.

The course also covers basic Change Impact Analysis and risk assessment to ensure that proposed actions are evaluated for potential operational and compliance risks before implementation.

Learning Objectives

By the end of the session, participants should be able to:

• Define root cause analysis (RCA) and explain its importance in corrective action and continual improvement processes.
• Differentiate between symptoms, immediate causes, and true root causes within operational and compliance contexts.
• Assess use cases for each tool and apply them to diagnose and resolve process challenges.
• Create a process for utilizing root cause analysis with your team.
• Correctly completing a Corrective Action Report.
